Care Insight Inc. SMS Terms and Conditions

By opting into or using the Care Insight Inc. text messaging service ("SMS Program"), you agree to these Terms and Conditions.

1. Program Details

Program Name: Care Management Members Outreach.
Company Name: Care Insight Inc.
Program Description: This SMS Program provides recurring, transactional, and informational text messages related to your enrollment in Care Insight Inc.'s care management services. Messages include, but are not limited to, appointment confirmations and reminders, scheduling updates, prescription refill reminders, and responses to support inquiries.
Message Frequency: Message frequency will vary based on your enrollment status and interaction with your care plan.

2. Consent and Opt-In

User Consent: By providing your mobile phone number and explicitly checking the consent box on the Care Insight Inc. website, you provide express written consent to receive recurring text messages from Care Insight Inc.
No Condition of Purchase: Consent to receive text messages is not a condition of purchasing any services or products from Care Insight Inc.

3. Cost and Carriers

Message and Data Rates: Message and data rates may apply to any messages sent to you from us and to us from you. If you have any questions about your text plan or data plan, please contact your wireless provider.
Carriers: Carriers are not liable for delayed or undelivered messages.

4. Opt-Out and Support

Opt-Out Instructions: You can cancel the SMS service at any time. To stop receiving all messages, simply reply STOP to any message you receive. After you text STOP to us, we will send you an SMS reply to confirm that you have been unsubscribed. You will no longer receive any messages from that number.
Support/Help Instructions: If you are experiencing issues or need assistance, you can reply HELP to any message you receive. Alternatively, you may contact our Customer Support at info@careinsight.com.

5. Privacy and Security Disclosures

Privacy Policy: Your information will be handled in accordance with our Privacy Policy.
Non-Sharing of Mobile Data: We will not sell, rent, or otherwise share your mobile phone number or opt-in data with unaffiliated third parties for marketing or promotional purposes.
Security Risk Disclosure: Text messages (SMS) are not inherently secure and may be viewed by anyone who has access to your mobile device. By opting in, you acknowledge that you have been informed of the potential risks of communicating via text message, and you accept those risks. We limit the amount of Protected Health Information (PHI) shared via text to the minimum necessary for care management.
